Activities Request
Please complete the request form below for your event or activity.

The request will be submitted to the Director of Operations/Office Manager for review & approval. Specific space may be requested; however, it is at the church's discretion which space(s) is available and best suited for the requested purpose and size of the group. The church clerk will advise the contact person whether their request has been approved.

Please provide a minimum of 30 days notice but no more than 45 days unless requesting weddings or conferences (or other large events). No plans or commitments should be made until approval is given.
Graphics For Ministry Events:
​​Not every graphic will be disseminated via all communication platforms. Completed graphics will be shared to the platform(s) determined by the social media team, or by Pastor Poindexter’s request.
ALL graphics/marketing materials are to be created ONLY by the social media/Multimedia Specialist.
Graphics will not be made for external/community events or activities.

Support From Other Ministries:
You will receive a confirmation email within 48 hrs. from the Office Manager or Director of Operations confirming as requested, confirming with modifications, or denying your space request. Prior to discussing details with support ministries, such as Hospitality or Audiovisual, your space request must be approved. Once approved, you must contact the support ministries to work through the details.
